Indications and Usage

A macrolide antibacterial indicated for mild to moderate infections caused by susceptible bacteria, including acute bacterial exacerbations of COPD, acute bacterial sinusitis, community-acquired pneumonia, pharyngitis and tonsillitis, uncomplicated skin and skin-structure infections, urethritis and cervicitis, and genital ulcer disease.

Dosage and Administration

The common adult regimen is 500 mg as a single dose on day 1 followed by 250 mg once daily on days 2 through 5, for a total of 1.5 g. Acute bacterial sinusitis may be treated with 500 mg once daily for 3 days, and certain sexually transmitted infections with a single 1 g or 2 g dose.

Warnings and Precautions

  • Serious and sometimes fatal allergic and skin reactions, including angioedema, DRESS, and Stevens-Johnson syndrome; discontinue if they occur
  • Severe and sometimes fatal hepatotoxicity; discontinue immediately at signs of hepatitis
  • QT prolongation and torsades de pointes; use caution in patients with known QT prolongation or on other QT-prolonging drugs
  • Clostridioides difficile-associated diarrhea and possible exacerbation of myasthenia gravis

Contraindications

Contraindicated in patients with known hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide, or with a history of cholestatic jaundice or hepatic dysfunction associated with prior azithromycin use.

Adverse Reactions

The most common adverse reactions are diarrhea, nausea, abdominal pain, and vomiting.

Storage and Handling

Store at 20°C to 25°C (68°F to 77°F), with excursions permitted to 15°C to 30°C (59°F to 86°F). Dispense in tight containers.

Film-coated 250 mg tablets supplied as cartons of blister cards containing 6 tablets per card.
