Indications and Usage

Indicated to reduce elevated intraocular pressure in patients with open-angle glaucoma or ocular hypertension who are insufficiently responsive to beta-blockers alone. It combines a carbonic anhydrase inhibitor with a beta-adrenergic blocking agent.

Dosage and Administration

Instill one drop in the affected eye(s) two times daily. If more than one topical ophthalmic product is used, administer the products at least ten minutes apart.

Warnings and Precautions

  • Dorzolamide is a sulfonamide; systemic absorption can cause the same serious reactions seen with sulfonamides, including Stevens-Johnson syndrome and toxic epidermal necrolysis; discontinue if signs of hypersensitivity occur
  • Timolol is systemically absorbed and may produce the cardiac and respiratory adverse effects of oral beta-blockers, including cardiac failure and bronchospasm, and can mask signs of hypoglycemia and hyperthyroidism
  • Use with caution in patients with hepatic or renal impairment and avoid abrupt discontinuation of beta-blocker therapy
  • Ocular effects include bacterial keratitis from contaminated containers and choroidal detachment reported after filtration procedures

Contraindications

Contraindicated in patients with bronchial asthma or history of asthma, severe chronic obstructive pulmonary disease, sinus bradycardia, second- or third-degree atrioventricular block, overt cardiac failure, cardiogenic shock, or hypersensitivity to any component.

Adverse Reactions

The most frequently reported adverse reactions were ocular burning and stinging, taste perversion (bitter or unusual taste), and blurred vision. Conjunctival hyperemia, superficial punctate keratitis, and eye itching were also reported.

Storage and Handling

Store at 20°C to 25°C (68°F to 77°F) and protect from light.

Supplied as a sterile ophthalmic solution in a white opaque plastic ophthalmic dispenser bottle with a controlled drop tip.
