Avesis and Other Vision Plans the smaller panels worth a look.

Beyond the four national names, a group of administrators runs vision benefits for Medicaid, CHIP and Medicare Advantage health plans and for smaller employers. Avesis is the clearest example. Here is how these plans work and what to check before you sign.

Patient checking out at an optical front desk

Quick answer

Many of these plans sit behind a health plan.

Avesis, a wholly owned subsidiary of Guardian, administers vision, dental and hearing programs for Medicaid, CHIP and Medicare Advantage as a subcontractor to managed care organizations, and also offers vision and dental benefits to employers. For a practice, the visible plan name is often the health plan, while credentialing, claims and fee schedules come from the vision administrator behind it.

Key takeaways

  • Avesis is owned by Guardian; the purchase was announced in December 2015.
  • Avesis subcontracts vision programs for Medicaid, CHIP and Medicare Advantage plans.
  • CAQH is Avesis's preferred credentialing application, and contracting is a separate step.
  • Medicare Advantage vision benefits may include routine exams and eyewear every 24 months, per CMS.
  • Read every regional plan's fee schedule and lab rules before you join.

Vision benefits inside Medicare Advantage.

Original Medicare excludes routine vision care, so Medicare Advantage plans often add it through a vision administrator.

Routine eye exams Not part of Original Medicare
Eyeglass frames May be covered once every 24 months
One pair of lenses or contact lenses May be covered once every 24 months

Questions to ask any regional plan.

A short checklist before you add a smaller panel.

Question Why it matters
Who administers the vision benefit? The health plan name on the card may not be the company that pays your claim
What are the exam and materials fees? Compare them with your cost per exam and your optical margins
Which labs are required? Plan labs can remove the optical's materials margin
How do claims and eligibility work? Portal, clearinghouse or paper changes front-desk time
How is credentialing handled? CAQH-based applications move faster when your profile is current
Practice pearl

Treat a regional panel like a new product line. Run one quarter of projected visits through its fee schedule and lab rules before you sign, and keep your CAQH profile current so applications move quickly when a plan does make sense.

Does Avesis use CAQH?

Yes. Avesis names CAQH as its preferred credentialing application.

Is contracting the same as credentialing?
