Specialty lens fits can be billed to medical insurance or to a vision plan's medically necessary contact lens benefit when the diagnosis and documentation support it. The fitting code, the material V code and the chart all have to line up.

Write the medical necessity statement at the fitting visit, while the topography and acuity are in front of you. Claims are rarely denied when the chart already proves the diagnosis.

Can I bill 92072 for every keratoconus visit?
No. It describes the initial fitting. Follow-up visits are billed with E/M or eye codes, and payers define initial fitting differently.
Which code fits a scleral lens for dry eye?
Ocular surface disease fits use 92071, billed per eye, with the scleral lens billed as V2531 per lens. Confirm with the payer.
What goes in box 19 for a scleral claim?
Lens type and the manufacturer or brand. Missing details can result in payment at the V2599 rate.
Do vision plans cover specialty lenses?
Many have a medically necessary contact lens benefit with published criteria. Check eligibility before the fit.
Do specialty lenses earn the participation credit?
No. Contact lenses and Rx are excluded. The 15% participation credit applies to eligible OTC orders on VisionRescue.com through RescueLink, such as scleral saline and GP solutions.
